- Please arrived with completed forms including Driver Information with DOT Medical Examination Report and Health History
- Note: A Department of Transportation physical will NOT replace a yearly wellness physical
- What You Can Expect At Your Visit:
- A practitioner will review your immunizations/shot record and medical history and conduct a general physical exam.
- Your physical exam will also include a urinalysis.
- Your Medical Examination Report form will be completed by your practitioner.
- A summary, receipt, and Medical Examiner’s Certificate will be provided.
- The summary of your visit will be sent to your primary health care provider (*you must give permission beforehand).
What DOT Physicals Consist Of
The DOT physical exam that is conducted includes the following:
- Blood pressure and pulse rate- Your provider will check your pulse and blood pressure to measure diastolic and systolic pressure levels.
- Vision- With or without corrective lenses, commercial drivers should have at least an acuity of 20/40 in each eye.
